The first time that you visit, you will be required to complete a series of new patient papers. In addition to basic information such as your name, age, date of birth, contact information, and place of employment, you will be asked for your dental insurance information, a brief health and dental history, and preferences regarding fillings. A form containing our privacy policy will be given to you for your personal records.

 

Additionally, during your first visit with Smiles Today Dental, our hygienist will ask you if you are currently experiencing any problematic issues. This is an important part of your exam since the hygienist will make note of the problem on your chart, check into it herself, and bring it to the attention of the dentist who is handling your first visit.

 

Dental Insurance and First Time Patients
